CS1070.10 - Case Management and Community Services

Case management is an essential aspect of community services work. In this subject, students will explore the history and development of case management within the Australian context. In this subject, students will also develop knowledge and skills in undertaking holistic social assessment, developing care plans in consultation with service users and their carers, coordinating services, monitoring and evaluating care plans, and transitioning service users to independent and interdependent living. This enables students to utilise the structured steps of case management and analysed how they are applied to meet the program requirements and service users' needs.
